Patients may also be eligible for financial assistance if uninsured, underinsured, or unable to pay out-of-pocket expenses for medically necessary care provided by Wyandot Memorial Hospital.

Wyandot Memorial Hospital provides financial assistance based upon need. To determine need, we review your household income and compare it to the Federal Poverty Level guidelines set by the U.S. Department of Health and Human Services. Wyandot Memorial Hospital participates with the Ohio Health Care Assurance Program and will provide medically necessary services at no charge to qualified Ohio residents. To qualify, you and your family’s income must be at or below the Federal Poverty Guidelines.
Financial Assistance applications can be obtained at the Patient Access Department located in the Main Hospital Entrance or at the Surgery Center Entrance. Financial Assistance application can also be downloaded and mailed along with the required documents to:
